How to request your child's medical records

Parents and legal guardians may ask for release of their child's health information (medical records) by filling out a request form and returning it to us. It usually takes seven to 15 working days from the time we get your request to the day your child's records are delivered to you or to the person you authorize to receive them. We have two request forms:

Please use this form to get your child's medical records or to send them to people other than school officials.

Please use this form when you want to share your child's health information with school officials.

You may fax, mail or deliver the form to us.

Seattle Children's provides health care for the special needs of children regardless of race, sex, creed, ethnicity or disability. Financial assistance for medically necessary services is based on family income and hospital resources and is provided to children under age 21 whose primary residence is Washington, Alaska, Montana and Idaho.
